 How does this work?

 At Simplex Health, our nutrition counseling is patient-centered, focusing on prevention and root cause resolution of health conditions. Our Dietitians address a wide range of health concerns, including weight management, chronic disease, symptom reduction, and preventative care. After you schedule an appointment, you will meet with one of our Registered Dietitians either virtually or in-person at one of our offices located throughout the greater Philadelphia region. 

 

What is an appointment like?

During the initial appointment, which will take approximately 60 minutes, you and your Simplex Health Dietitian will get to know each other and establish your motivations for becoming healthier. At this time, your Dietitian will also gather information regarding your past medical history, eating habits, stress levels, sleep habits, and more in order to treat you as a whole person rather than treating your disease.

 

 You and your Dietitian will create short- and long-term goals for your health journey along with individualized nutrition solutions.

 

 At the end of your initial appointment, the Dietitian will assist you in scheduling a follow-up appointment to monitor your progress and make changes as needed. Before the follow-up appointment, you can always reach out to us over the phone or email or chat through Healthie if you are struggling or have any questions along the way.

 

How does a virtual appointment work?

Once you complete all your intake paperwork and required fields, you will gain access to your appointment. When you login to Healhie there is a banner/button across the top of the screen. Ensure you have a strong internet connection and give yourself an extra few minutes especially for your first appointment to make sure everything works. 

  

How long is an appointment?

Initial 1 hour, follow up 30-45 minutes, sometimes an hour. 

 

What if an office is far away?

Virtual appointments are available. Ask if your insurance allows for virtual sessions.

 

How often do I need to be seen?

This all depends on your initial conversation with your RD. Generally, we like to see new patients every 7-10 days.

 

What should I bring?

  •  Your most current blood work/labs – you can request a copy of these from your doctor – the office can fax them to 484-930-0063
  •  A list of medications and supplements you are currently taking.
  •  A referral form, if needed. This form is always needed for Medicare patients; many other insurance companies don’t require this, but double check with your carrier.
  •  If you have diabetes, bring your blood glucose log.
  •  Your list of questions to ask during the visit.
  •  Insurance Card(s) and ID.
  •  Your Copay, if applicable, this can be check or credit card.

 

Cancellation policy?

24 hours notice. If you are unable to keep your appointment, please contact us at 877-842-2425 as soon as possible.


What is covered with insurance?

We work in-network with all major plans, most plans allow for free preventative sessions with a dietitian. Additional appointments may be available with a diagnosis. Is your insurance accepted? CHECK HERE


What about new patient forms?

Our initial appointment/new patient forms are housed in our HIPAA-secure portal, Healthie. When you become a new patient, you will receive a welcome email where you can create a login and begin the intake process! Everything is digital. This saves time prior to your appointment and saves some trees.

We ask that you take your time going through the forms. They are comprehensive but important to get the full picture.


What is Healthie?

Healthie is our HIPAA-secure online that provides our patients access virtual appointments, a place to complete intake forms, upload health documents, track meals and activity and chat with your Dietitian for questions in between sessions. 

 

What if I already have a diagnosis?

If you have a current medical condition, our Lead Dietitian will review your lab results, medications, and other specific information related to your condition with our Medical Director to identify the potential root cause and develop a course of action.
